Access ID Cards - Dental & Vision Plans

Access ID Cards

Your Voluntary Group Benefits Portal

MetLife Dental ID Cards

Your dental provider will need an ID card to process your dental claims. Your dental ID card contains the information the providers will need such as your carrier name, subscriber name, group number, network name, plan information access to verify benefits and claim information.

It is important that you take your card with you when you visit any dental providers. You may log on to www.metlife.com/dental to order a new ID card and also access a temporary
​ID card. You will be required to register first to access this system.

MetLife Vision ID Cards

Your vision provider will also need an ID card to process your vision claims. Your vision ID Card contains the information the providers will need to verify benefits and process claim information.

To order log on to www.metlife.com/vision to order a new card and also access a
​temporary ID card. You will be required to register to access this system.
