Dental Providers

You may use your own dental provider or select an in-network provider through MetLife and
​take advantage of negotiated group provider rates which will save you money. The MetLife
dental network is the (MetLife PDP Plus PPO). You can access them at You may also call MetLife Customer Service at 800-ASK​-

ID Cards

Your dental provider will need an ID Card to process your dental claims. Your dental ID Card contains the information the providers will need such as your carrier name, subscriber name, group number, network name, plan information access to verify benefits and claims information. It is important that you take your card with you when you visit any dental providers. You may log on to to order a new card and also
​access a temporary ID Card. You may also call MetLife at Toll Free: 1-800-ASK-4-MET or 1-800-275-4638.


Enrollment in the group dental program is not automatic and you will need to enroll within 30 days after your hire date or experience a qualifying event or annual open enrollment which
​will take place in the latter part of the 4th quarter every year. Notices for open enrollment will be distributed early to give you time to enroll. Enrollment forms are included in the "Forms Library" for your convenience.

The program offers two separate plans; a high option ( with Orthodontia ) and a second option without Orthodontia.
