You may use your own vision provider or select an in-network provider through MetLife and
take advantage of negotiated group provider rates which will save you money. The
MetLife vision network is the (MetLife Vision PPO Network). You can access them at
www.metlife.com/mybenefits. You may also call the MetLife Customer Service
Center at 800-275-4638.
Click "here" to access the 2018 Group Benefits Guide. On page # 10 you will find the details
on the plan design you may enroll in. On page # 19 you will find the employee and dependent monthly rates. You will also find a plan certificate and plan summary in the "Forms Library" page for further plan details.
Interested in online access for MetLife? Registering is easy and you can access your complete benefits, plan details, providers search engine, claim processing information, order and print temporary ID cards. Log on to www.metlife.com/mybenefits to create your personalized home page access.
Your vision provider will need an ID Card to process you vision claims. Your vision ID Card contains the information the providers will need such as your carrier name, subscriber name, group number, network name, plan information access to verify benefit information. It is important that you take your card with you when you visit any vision providers. You may log
on to www.metlife.com/mybenefits to order a new card and also access a temporary ID Card. You may also call MetLife Customer Service at Toll Free: 1-800-275-4638.
Enrollment in the group vision program is not automatic and you will need to enroll within 30 days after your hire date or experience a qualifying event or annual open enrollment which will take place in the latter part of the 4th quarter every year. Notices for open enrollment will be distributed early to give you time to enroll. Enrollment forms are included in the "Forms Library" for your convenience.
